Community Connections of California is a nonprofit organization dedicated to empowering individuals and strengthening communities through compassion, collaboration, and holistic support. We believe that meaningful connection is the foundation of healing and growth.
Our programs help individuals—particularly those facing barriers such as justice involvement, homelessness, or limited access to care—rebuild their lives through stable housing, enhanced care management, and community-based resources. By fostering partnerships across health, social service, and community systems, we create pathways toward stability, independence, and long-term success.
