Job Title: ENHANCED CARE MANAGER

Job Summary:  

Our Enhanced Care Managers provide one-on-one support to Medi-Cal members with complex medical and social needs. ECMs help clients access healthcare, housing, and community resources while building trusting relationships that promote healing and independence. Ideal candidates are compassionate, organized, and passionate about helping others overcome barriers and achieve stability.

Key Focus Areas:

• Care coordination and advocacy
   
• Linking clients to medical, behavioral health, and social services
   
• Supporting members through transitions (hospital, incarceration, housing)
   
• Promoting wellness, independence, and self-sufficiency

Job Title: HOUSING NAVIGATOR

Job Summary:  

Housing Navigators help individuals and families find safe, affordable housing and maintain long-term stability. HNs work directly with clients, landlords, and community partners to secure housing, coordinate move-ins, and connect members to ongoing support. Ideal candidates are resourceful, empathetic, and dedicated to helping people rebuild their lives.

• Locating and securing housing opportunities
   
• Assisting with rental applications and move-in support
   
• Providing transportation and case management
   
• Promoting housing stability and community connection
