Enhanced Care Management
At Community Connections of California, our Enhanced Care Management (ECM) program provides whole-person, compassionate care coordination for individuals with complex medical and social needs.
We understand that healing and stability require more than medical treatment—it takes connection, trust, and consistent support.
Our ECM team partners closely with each member to identify barriers, build individualized care plans, and ensure access to the right services at the right time. Through collaboration with healthcare providers, housing agencies, and community partners, we help members achieve better health outcomes and long-term independence.

Who qualifies?
ECM services are available to Medi-Cal members who have complex needs and would benefit from extra, coordinated support. Individuals may qualify if they:
- Are experiencing homelessness or housing instability
- Have a serious mental illness or substance use disorder
- Are frequent users of hospital or emergency services
- Are transitioning from incarceration or institutional care
- Are pregnant or parenting with medical or social risk factors
- Are youth with complex physical, behavioral, or developmental needs
- Face multiple barriers to care, such as transportation, food insecurity, or lack of support
