Partnership health plan

We serve Medi-Cal members with complex medical, behavioral, and social needs through whole-person care coordination. Our populations of focus include individuals experiencing homelessness, those with serious mental health or substance use needs, high utilizers of emergency or hospital services, people transitioning from incarceration or institutional settings, and children, youth, and families with complex care needs.
